Mammary augmentation surgery is one of the world`s most popular aesthetic surgeries. Breast implants cosmetically improve appearance and self image of patients with small breasts. Patients with flat buttocks may get more gluteus volume by means of silicone implants. Silicone breast implants have been used since 1960 in millions of women around the world. Because of the FDA moratorium against silicone filled implants, saline filled implants were the only authorized implants in USA. In 2000 the Institute of Medicine concluded that silicone gel implants do not increase the risk of breast cancer or autoimmune diseases. In 2005, FDA allowed some brands of silicone implants to be used in patients for cosmetic or reconstructive reasons. In general terms there are two types of breast implants worldwide accepted, saline and silicone gel filled. Saline filled implants may have a smooth or textured outer surface made of solid silicone. The textured surface has a lower incidence of tight scar formation around it or capsule retraction. The implant is filled through a special designed one way valve with a silicone tube.
This implant allows the surgeon to do smaller skin incisions because the deflated implant is small and may be rolled up before inserting it into the breast.
Once inside the breast, the surgeon fills the implant with saline solution until it reaches the adequate volume.

The final results with these implants are definitive. Sometimes the valve does not work or the saline evaporates through the implant wall, the breast looses volume and shows wrinkles around the outer circumference.
Bilateral partially deflated implants In these cases, the implant should be changed. Because it is a normal saline solution, it is non toxic and does not have any secondary effects. Silicone filled implants, as the saline ones, may have smooth or textured surface.

The silicone gel may be soft or cohesive. Implants filled with soft gel do not maintain a fixed shape and they change as the breast moves. Cohesive gel filled implants have a fixed shape and movement or pressure on the breast do not change them.
Soft silicone gel Cohesive silicone gel
If the implant filled with soft gel is torn, gel diffuses out into the capsule that surrounds the implant or migrates outside the capsule into the breast tissue. If the cohesive gel filled implant is torn, silicone keeps its semi-compact shape and does not bleed into the capsule.

There are two different implant shapes, anatomical and round. Anatomical implants are used mostly for breast reconstruction after mammary cancer, and round implants are commonly used for aesthetic purposes. |

Round implants are different too. Depending on the chest circumference and the breast width , the surgeon will choose between low or high profile implants. With the same volume, the implant’s projection can vary when it is placed beneath the breast according to different arrangements in its dimensions. |

Buttock implants, are made of semi-solid silicone gel to improve their strength to support constant gluteus pressure and frequent movement. The implant may have a smooth or textured outer surface and is flatter than the breast implant. The shape varies from round to anatomical. Once inside, beneath the gluteus muscle, the body creates a capsule like a scat tissue that surrounds the implant, as happen with the breast implants. The results are definitive and the patient should avoid any buttock injection for life. |
